| Description: | Provides hands on, in-home care to each assigned patient. Supervises and teaches team members, including RN’s, LPN’s and HHA’s in the efficient and effective delivery of quality patient care. |
| Duties: | -Accepts patient care reports and patient assignments from the Primary Nurse and/or Case Manager for all patients assigned. Coordinates with the Primary Nurse and/or Case Manager timely completion of assigned visits and submission of the required documentation.
-Provides frequent communication to the Case Manager, physicians, other team members, etc., and the patient/family in efforts to coordinate changes in patient care and assures appropriate follow-up and supporting documentation is done.
-Assigns, develops, supervises and evaluates personal care services provided by the staff in accordance with organizational policies, procedures and processes and Medicare/State regulations. Develops and revises, when indicated, the personal care-care plan for each patient receiving care.
-Makes assigned patient care visits in accordance with physician orders. Completes supporting documentation in an accurate and timely manner, including but not limited to; visit report, OASIS assessment, care plan/plan of care revisions, medication profile, progress notes, revisions to the plan of treatment for recertification, discharge/transfer summary, etc.

| Qualifications: | -Registered Nurse, currently licensed in the state of Tennessee.
-Current valid Tennessee driver’s license.
-Must be able to provide direct patient care and take on-call.
-Working knowledge of the home health care program and previous experience providing home health visits preferred.
