|Description:||The RN/Director of Patient Care (DOPC) coordinates total patient care from pre-admission to discharge and supervises the community-based office of Home Health Care of Middle Tennessee.|
|Duties:||The DOPC is responsible for:|
-Maintaining the staffing and weekly nursing schedule necessary for providing the highest level of patient care.
-Supervising and instructing all personnel, including RN's, LPN's, Home Health Aides, and office staff.
-Overseeing each step of the patient referral process to ensure all patient needs are met in accordance with the physician orders.
-Performing pre-admission visits, post-hospital visits, on-call, and/or emergency visits when necessary.
-Monitoring all admissions/patient records for proper and timely staff documentation and adherence to all company, government, and insurance guidelines.
-Maintaining the effective flow of communication between nursing staff and all other departments to optimize patient care.
-Planning and implementation of the office budget for provision of patient services.
-Ensuring staff adherence to all company policies with the timely resolution and documentation of any issues.
-Registered Nurse (RN), licensed in Tennessee
-Previous Home Health experience required
-Thorough knowledge of the framework, organization, and function of the Home Health program
-Thorough understanding of the principles and methods involved in effectively hiring, supervising, and managing staff.
